Program Information
• Benefits include nutrition counselling, breastfeeding support, food benefits, and money to farmers markets.
• Application may be done online or over the phone, followed by an in-person appointment.
• You will need to return for recertification appointments.
Eligibility
• Residents of Minnesota
• Attend in-person appointments or provide certified and up to date health information for children who cannot attend
• Meet income eligibility or qualify through adjunctive program
• Be a member of one of the eligible categories:
• Pregnant Women
• Women within 6 months of birth or pregnancy
• Breastfeeding women up to 1 year postpartum
• Infants 0-12 Months
• Children 1-5 Years
Income Limits
Application Requirements
• Proof of identity for each person certified
• Proof of address for the household
• Proof of income OR
• Proof of participation in an adjunctive program
